Qualis Health Safer Transitions Webinar

Within 30 days of being discharged, one in five Medicare patients returns to the hospital—often with serious preventable complications. Improving transitions of care requires a collaborative community approach.

Qualis Health is partnering with the Washington State Hospital Association to bring statewide care transitions learning opportunities via face-to-face meetings and webinars for any interested providers, community-based organizations and stakeholders. Assistance will focus on Washington state-specific healthcare issues and opportunities.

These action-oriented sessions will include evidence-based interventions, opportunities to share ideas with peers, and specific ideas to test and implement in your setting.
