• Enhanced Care Coordination Beginning July 2012
  Qualis Health Care Coordinators will be providing discharge planning support to Alaska Medicaid residential treatment facilities located within and out of the State of Alaska. Care coordination services will include conducting family assessments of resources needed and discharge readiness with Alaska Medicaid recipients' families.
