2015 Qualis Health Washington Quality Award Winners Announced

Qualis Health Recognizes Seven Washington Organizations for Improvements in Healthcare

Seattle, WA - Qualis Health, one of the nation's leading population health management organizations, will present the Washington Awards of Excellence in Healthcare Quality on May 13. Seven healthcare organizations from across the state of Washington will be recognized for their achievements in improving healthcare quality and outcomes.

"Our changing healthcare landscape requires increasingly vigorous efforts to improve healthcare delivery and health outcomes in Washington and across the nation. The Qualis Health Washington Quality Awards celebrate organizations that have transformed healthcare delivery for their communities—making a real difference for their patients and their families," said Jonathan Sugarman, MD, MPH, President and CEO of Qualis Health. "Their achievements exemplify the aims of improved healthcare: better quality of care at a lower cost."

The award recipients will be recognized during a ceremony at the 2015 Northwest Patient Safety Conference on May 13 in Lynnwood, WA. Dr. Sugarman will present the awards.

The 2015 Awards of Excellence in Healthcare Quality are awarded to the following organizations:

• Award of Excellence: Post-Acute/Long-Term Care
  Puyallup Nursing and Rehabilitation Center, Puyallup, for their project "Reducing Prevalence of New or Worsening Pressure Ulcers in Nursing Home Residents"
• Award of Excellence: Critical Access Hospital
  Whitman Hospital & Medical Center, Colfax, for their project "Readmission Reduction"
• Award of Excellence: Outpatient Services
  Northwest Kidney Centers, Seattle, for their "Improving Kidney Transplantation Initiative"
• Award of Excellence: Hospital
  UW Medicine - Harborview Medical Center, Seattle, for their project "Improving Chronic Care and Reducing Readmissions in a Highly Vulnerable Heart Failure Safety Net Population"
• Award of Excellence: Hospital
  UW Medicine - Harborview Medical Center, Northwest Hospital & Medical Center, University of Washington Medical Center and Valley Medical Center, Seattle, for their project "An Enterprise Approach to Evidence-based Performance Improvement for Central Venous Catheter (CVC) Associated Complications"
• Award of Excellence: Hospital
  Virginia Mason Medical Center, Seattle, for their project "Venous Thromboembolism Prophylaxis Journey"
• Award of Excellence: Hospital
  Yakima Valley Memorial Hospital, Yakima, for their project "Meds to Beds; Your Prescription Delivered to Your Bedside"
