State of Alaska, Department of Health and Social Services, Division of Health Care Services - Coordinated Care Initiative (AMCCI)

The Alaska Medicaid Coordinated Care Initiative (AMCCI) is designed to assist Alaska Medicaid members who need help navigating the healthcare system and can benefit from case management services. Because these members may have trouble navigating the healthcare system, they may become frequent utilizers of the hospital emergency room for issues that may be better treated in a primary care setting.

The Division of Health Care Services has contracted with Qualis Health to provide case management services to high-utilizing AMCCI members. Qualis Health works closely with the medical and social service community to support and help coordinate care for these patients.

The AMCCI program is a pilot project. The AMCCI is a voluntary program.

Goals of the AMCCI

The primary objective of the AMCCI is to promote quality, cost-effective health outcomes by ensuring that coordinated medical services are provided in the most appropriate setting. Other goals include focusing on prevention and enhanced integration of primary medical care and behavioral health services. And most importantly, the AMCCI hopes to make a difference by improving the health and quality of life for Alaska Medicaid members.

How We Help

From our Anchorage office, Qualis Health provides in-person case management for Alaska Medicaid members in the Anchorage area with high emergency room use—some with complex medical conditions, behavioral health conditions and/or social issues. Our staff is composed of Alaskans who provide services specifically tailored to the needs of Alaska residents. We are privileged to perform this vital work.

Overview of Case Management Activities

Qualis Health performs one-on-one, in-person case management services. These services include:

  • Initial health screenings
  • Clinical assessments
  • Targeted interventions, which may include:
    • Primary care coordination
    • Coordination with behavioral health and social service providers
    • Assistance with stable housing plan and food plan
    • Medication reconciliation
    • Prevention and self-management support

For utilization review, case management and related activities, we are continuing to process requests and answer calls via our toll-free numbers during standard business hours. We have business continuity plans in place to serve our clients, health care providers, patients and communities while also protecting our staff and minimizing the spread of the illness.
