Safety Net Medical Home Initiative Featured in Agency for Healthcare Research and Quality Publication

Seattle, WA - The Qualis Health Safety Net Medical Home Initiative (SNMHI) has been featured in the Agency for Healthcare Research and Quality's (AHRQ) Case Studies of Leading Primary Care Practice Facilitation Programs: Overview. The SNMHI is one of four case studies that highlight innovative practice facilitation approaches and activities. "We're pleased that AHRQ has identified the SNMHI as a leading practice facilitation program," says Jonathan Sugarman, MD, MPH, Qualis Health President and CEO. "We hope that our work will continue to contribute to the efforts of others to support primary care transformation."

The goal of the SNMHI is to help practices redesign their clinic and administrative systems to improve patient health by supporting effective and continuous relationships between patients and their care teams. The SNMHI is sponsored by The Commonwealth Fund and is adminstered by Qualis Health and the MacColl Center for Health Care Innovation at the Group Health Research Institute. SNMHI is also featured in Lessons Learned from Leading Models of Practice Facilitation.
